NYS Infertility Demonstration Program
Are You Eligible To Receive a Grant from New York
State for your IVF treatment?
If you possess insurance but do not have infertility coverage or have
exhausted your infertility benefits, you may qualify to
receive financial assistance for your in vitro
fertilization (IVF) treatment through the New York State
Infertility Demonstration Program.
In 2002, New York State passed legislation to provide this
Infertility Demonstration Program, which is allocated to select
centers meeting high standards of IVF
success rates and patient volume.
Reproductive Medicine Associates of New York is among these select
centers selected to participate in the infertility grant to provide
funding to eligible patients in need of
infertility treatment. The grant helps subsidize payments
for most services associated with IVF, including
patient assessment (1 allowed per patient for the life of the grant) and
the IVF cycle (maximum of 2 complete cycles for the
life of the grant). The amount of treatment funding provided to the
eligible individual/couple is based on the individual/couple's combined
household income; coverage can range from 2.5% up to 97.5% of
IVF treatment costs.
Infertility treatment services that may be covered
within the cycle are:
Exclusions from this treatment subsidy include
ovum donation
fees, ovulation induction, general gynecologic care,
preimplantation genetic diagnosis (PGD), and surgery.

intrauterine insemination (IUI)
b) diagnosed tubal factor, or c)
severe male factor infertility? |
Patients are ineligible for the
New York State Infertility Demonstration Program if
they have an abnormal day 3 FSH, have a medical contradiction to
pregnancy, have had 3 or more failed or cancelled IVF
cycles, are self-insured, or possess Medicare or Medicaid insurance.
